Vitamine B12 is eigenlijk een verzameling van vier verschillende kobalthoudende moleculen die elk een specifieke en cruciale rol spelen in het lichaam.

Er zijn veel verschillende factoren die ME/CVS en andere multisysteemziekten kunnen uitlokken. Aangezien de symptomen van deze aandoeningen gelijkenissen vertonen, vermoeden onderzoekers dat deze patiënten dezelfde biochemische afwijkingen hebben.

Als deze gemeenschappelijke biochemische afwijkingen gevonden worden, kan een doeltreffende behandeling de gezondheidstoestand van de patiënten sneller doen verbeteren.

 

De zoektocht naar een unieke theorie met betrekking tot ME/CVS/FM heeft geleid tot twee modellen die wetenschappelijke ondersteuning genieten. Uit onderzoek blijkt dat het verband tussen ME/CVS/FM, de ziekte van Lyme, Meervoudige Chemische Sensitiviteit en Golfoorlog syndroom en een vitamine, die al even complex is en ook verkeerd begrepen wordt, voor veel patiënten een sleutel kan zijn tot een gezonder lichaam.

Het verband tussen vitamine B12 en ME/CVS/FM

In hun zoektocht naar gemeenschappelijke biochemische patronen bij ME/CVS/FM-patiënten hebben onderzoekers talrijke gelijkenissen gevonden tussen een vitamine B12-tekort en de symptomen van ME/CVS/FM en andere multisysteemaandoeningen.

Tal van gerenommeerde onderzoekers en artsen die zich buigen over ME/CVS/FM, zoals Paul Cheney, Charles Lapp, Kenny De Meirleir, Jacob Teitelbaum en Martin Pall, beschouwen vitamine B12 als een heel belangrijk onderdeel van het behandelplan. (1-6)

Een studie vond zelfs verhoogde energieniveaus terug bij mensen die geen vitamine B12 tekort hadden, maar toch de vitamine toegediend kregen. (7) Bij een toediening van B12 (2500-5000 mcg) elke twee of drie dagen werd een verbetering opgemerkt bij 50% tot 80% van een groep mensen met ME/CVS. De grootste verbetering werd gezien nar verschillende weken toediening van vitamine B12. (8)

Symptomen van een vitamine B12-tekort

Vitamine B12 is noodzakelijk voor de aanmaak van rode bloedcellen, de myelineschede rond zenuwcellen en DNA. (9-12) De verschillende vormen van vitamine B12 zijn nodig voor talrijke andere fysische functies. Een tekort kan grote gevolgen hebben.

  • Energie: zelfs een klein tekort aan vitamine B12 kan de oorzaak zijn van bloedarmoede, vermoeidheid, kortademigheid en krachteloosheid. (9,10,13)
  • Zenuwstelsel: een vitamine B12-tekort kan neurologische veranderingen veroorzaken zoals gevoelloosheid en tintelingen in handen en voeten, (13,14) evenwichtsproblemen, depressie, verwardheid, geheugenstoornissen en symptomen die gelijken op de symptomen van de ziekte van Alzheimer.(15) Langdurige tekorten aan vitamine B12 kunnen een blijvende beperking van het zenuwstelsel tot gevolg hebben. (16,17,18,70,71)
  • Maag- en darmstelsel: een vitamine B12-tekort kan de oorzaak zijn van verminderde eetlust, constipatie, diarree of afwisselend constipatie en diarree (ook prikkelbaar darmsyndroom genoemd), gewichtsverlies en buikpijn.(9,10,13)
  • Immuunsysteem: vitamine B12 is nodig voor het normaal functioneren van de witte bloedcellen. (19) Studies tonen ook aan dat B12 helpt om de Natural Killer-T cellen te reguleren (20) en voorkomt schade aan chromosomen. (21)
  • Cardiovasculair systeem: vitamine B12 helpt bij de omzetting van homocysteïne in methionine. Een te hoog gehalte aan homocysteïne is een bekende risicofactor voor hartaanvallen, beroertes en tromboses. Zonder adequate B12 gehaltes, gaan de homocysteïne gehaltes doorgaans stijgen. (22-34)
  • Zintuigen. Degeneratieve veranderingen in het centraal zenuwstelsel die veroorzaakt worden door een B12 tekort kunnen ook invloed hebben op de optische zenuwen, wat resulteert in blauw-geel kleurenblindheid. (35)
  • Andere symptomen van vitamine B12 tekort zijn een pijnlijke mond of tong. (36)
  • Bij zuigelingen en kinderen zijn tekenen van vitamine B12-tekort onder andere groeiachterstand, bewegingsstoornissen, vertraagde ontwikkeling en megaloblastische anemie. (37)

Is ME/CVS/FM gelijk aan een tekort aan vitamine B12?

Symptomen die wijzen op een tekort aan vitamine B12 tonen veel gelijkenissen met symptomen van ME/CVS/FM. Toch blijkt uit onderzoek dat CVS en verwante multisysteemziekten niet louter veroorzaakt worden door een tekort aan vitamine B12.

Er wordt verondersteld dat twee opkomende theorieën van biochemische afwijkingen sterk betrokken zijn bij ME/CVS en andere multisysteemziekten. In feite kunnen elk of beide modellen van biochemische afwijkingen een centrale rol spelen in ME/CVS. Interessant is dat ze allebei aan een bepaalde vorm van vitamine B12-tekort gerelateerd zijn.

Het stikstofoxide/peroxynitrietmechanisme (“No, Oh No!”) bij ME/CVS/FM


ME/CVS/FM kan uitgelokt worden door veel verschillende factoren. De ziektesymptomen worden voorafgegaan door een virale, bacteriële of andere infectie, een fysiek of psychologisch trauma, chemische blootstelling of andere stressfactoren. (38-41,43,48,50)  Op dit vlak vertoont ME/CVS gelijkenissen met fibromyalgie (FM) (44,45,48,50), de ziekte van Lyme (47), Meervoudige Chemische Sensitiviteit (46,48) en Gulf War Syndrome (40-42, 48-49).

Volgens de theorie van gerenommeerd onderzoeker dr. Martin Pall, PhD, in detail uitgelegd in zijn boek Explaining ‘Unexplained Illnesses’, hebben ME/CVS en andere multisysteemziekten overlappende symptomen en initiële triggers (uitlokkende factoren) omdat ze een gemeenschappelijke metabole oorsprong hebben, een ‘vicieuze cirkel’ gerelateerd aan een overproductie van stikstofmonoxide (NO) en peroxynitriet (ONOO-). (51)

Gezien de chemische afkortingen voor deze twee substanties (NO- en ONOO-) noemt Dr. Pall deze vicieuze cirkel de “No, Oh No” cyclus, en hij presenteert solide bewijsmateriaal voor deze cyclus als een onderliggende oorzaak van ME/CVS en andere multisysteemziekten.

Dr. Pall heeft vastgesteld dat vrijwel elke bekende initiator van ME/CVS en andere multisysteemziekten een stijging veroorzaakt bij hetzij stikstofoxide (NO) (52-62) of bij hetzij het superoxide radicaal (O2-) of bij beiden. (54,57,73) Deze twee moleculen reageren snel en vormen peroxynitriet (ONOO-), (67-72,79,81,115,117,121), een krachtige oxidant die schade kan berokkenen aan tal van biologische moleculen.(54,68,69,74-92)

Peroxynitriet (ONOO-) reageert via verschillende mechanismen om O2- en NO- te regenereren. Op deze manier houdt de NO-/ONOO- cyclus zichzelf in stand, en een vicieuze cirkel is in gang gezet. (93-96) Tenzij er iets tussenkomt om de cyclus te doorbreken, is het mogelijk dat deze biochemische “eindeloze lus” zich voor onbepaalde tijd kan voortzetten.

Deze theorie krijgt steeds meer bijval en wordt beschouwd als een belangrijke onderliggende afwijking bij ME/CVS/FM en andere multisysteemziekten zoals de ziekte van Lyme, Meervoudige Chemische Sensitiviteit en Gulf War Syndrome. Deze eindeloze NO-/ONOO- cyclus is ook in verband gebracht met verhoogde pijnperceptie.   • Stikstofmonoxide (NO-) is een “boodschappermolecule” die van nature in het lichaam voorkomt. Het is een pro-oxidant en vrij radicaal. NO kan zowel nuttig als toxisch zijn, afhankelijk van de mate waarin en de plaats waar het vrijkomt. (141-145-,223) Het is bekend dat stikstofoxide een rol speelt bij de bloeddrukregulatie, bloedklonteren, immuniteit, spijsvertering, de zintuigen (zicht en reuk), en mogelijk het geheugen. Abnormale hoeveelheden NO- kunnen een rol spelen bij ziektes zoals atherosclerose, diabetes, beroerte, hypertensie, impotentie, septische shock, en langdurige depressie. (52,145) Bij ME/CVS/FM en verwante multisysteemziekten, suggereert onderzoek dat overdadig NO- een primaire bijdrage kan leveren aan langdurig energietekort en immuundisfunctie. (101,141-142,223)
  • Superoxide (O2-) is een krachtig vrij radicaal. Net zoals stikstofoxide (NO-), heeft O2- onafhankelijke schadelijke effecten wanneer het in overmaat aanwezig is. Superoxide reageert met NO- om ONOO- te vormen.
  • ONOO- (peroxynitriet) is een krachtig oxidant dat aangemaakt wordt wanneer NO en O2-met elkaar reageren. ONOO- beschadigt cellen door middel van verscheidene mechanismen. ONOO- regenereert NO en O2-aan de hand van verschillende andere mechanismen. Op die manier ontstaat er een vicieuze cirkel waarbij schade steeds meer schade aanricht.

Dr. Bell is een van de eerste artsen die ME/CVS erkent als een medische aandoening. In zijn boek Cellular Hypoxia and Neuro-Immune Fatigue stelt hij voor dat cellulaire hypoxia (verminderde zuustof naar de cellen) de onderliggende factor kan zijn bij ME/CVS en verwante multisysteem ziekten. (146) Dit is consistent met de NO-/ONOO- theorie, omdat schade van velerlei aard resulteert in verminderde zuurstoftoevoer naar de cel, dus een potentieel begin van zo’n destructieve eindeloze cyclus.

Hydroxocobalamine doorbreekt het stikstofoxide/peroxynitrietmechanisme (de NO-/ONOO- cyclus)


Hydroxocobalamine (cobinamide) is een specifieke vorm van vitamine B12, een krachtige stikstofmonoxide vanger en de enige vorm van vitamine B12 die de stikstofmonoxidemolecule kan neutraliseren.

Hydroxocobalamine is net die vorm van vitamine B12 die in staat is om het stikstofoxide/peroxynitrietmechanisme, dat oxidatieve schade berokkent, te doorbreken. (147-149)

De methylatiecyclus en ME/CVS/FM

Het methylatieproces is een biochemisch proces dat nodig is bij de aanmaak van DNA, RNA, fosfolipiden (de myelineschede van zenuwen), neurotransmitters, bijnierhormonen en meer dan 100 enzymen. Het methylatieproces zorgt ook voor tal van ontgiftingsreacties. (150-157)

Defecten in het methylatieproces worden als tweede mechanisme naar voren geschoven dat ME/CVS/FM in de hand werkt. Het onderzoek van Dr. Rich van Konynenburg, PhD, heeft bijgedragen aan de ontwikkeling van een theorie waarbij er een ingewikkelde wisselwerking is tussen de methylatiecyclus en ME/CVS. (158)

Bij een defect in de methylatie

  • vermindert het ontgiftingsvermogen en de productie van cellulaire energie,
  • neemt de productie van serotonine, dopamine, melatonine en andere neurotransmitters af,
  • verlaagt de productie van bijnierhormonen en
  • verhogen de waarden van het toxische homocysteïne.(159-163)

Merk op:

  • De overlapping tussen de NO-/ONOO- cyclus en de methylatiecyclus waar overdadig NO- de methione synthase, een cruciaal enzyme in de methylatiecyclus, blokkeert.
  • De methylcobalamine vorm van vitamine B12 is een noodzakelijk nutriënt in de methylatie cyclus. Als een onderdeel in de methylatiecyclus ontbreekt, dan faalt de volledige cyclus.

 

Welke vorm van vitamine B12 is de beste?

Wat algemeen bekend staat als vitamine B12 is in feite een verzameling van vier verwante maar verschillende kobalthoudende moleculen. Elke vorm heeft een specifieke functie in het lichaam:

  • Hydroxocobalamine:

    een unieke vorm van vitamine B12 die essentieel is om een verhoogde concentratie van stikstofmonoxide (NO), de voorloper van peroxynitriet (ONOO-), te verwijderen. (147-179,172-176) Hydroxocobalamine (en methylcobalamine) speelt ook een belangrijkere rol dan cyanocobalamine bij de behandeling van neurologische aandoeningen. (168)

    Hydroxocobalamine helpt ook bij ontgifting, vooral cyanideontgifting. Cyanide gehaltes zijn typisch verhoogd bij rokers, mensen die voedsel eten dat cyanide bevat (zoals maniok) en mensen die zeker metabolische defecten vertonen. Een teveel aan cyanide in de weefsels blokkeert de omzettin van cyanocobalamine in methylcobalamine of adenosylcobalamin. In zo’n geval is hydroxocobalamine de beste vitamine B12 keuze. (169-171) Hydroxocobalamine is goedgekeurd door de FDA als behandeling voor cyanide vergiftiging. (214)

  • Methylcobalamine:

    volgens veel onderzoekers de meest actieve vorm van vitamine B12 (177-179). Het is een noodzakelijke voedingsstof in het methylatieproces. (179-186) Methylcobalamine beschermt het zenuwstelsel tegen het schadelijk effect van glutamaat op neuronen (het beschermt de corticale neuronen tegen NMDA receptor gemedieerde glutamaat cytotoxiciteit). (187-188) en het heeft een gunstig effect op de regeneratie van zenuwcellen. Het heeft ook een bewezen positief effect op de slaapkwaliteit en bij het slaapverkwikken, evenals een verhoogd gevoel van welzijn, concentratie en alertheid. (190)

  • Adenosylcobalamine (dibencozide):

    ook een actieve vorm van vitamine B12; is essentieel voor het energiemetabolisme (191), en voor de normale vorming van de myelineschede en de synthese van nucleoproteïne. Een tekort eraan kan wijzen op een degeneratie van de zenuwen en het ruggenmerg. (192-193)

  • Cyanocobalamine:

    de meest gebruikte vorm van vitamine B12 in voedingssupplementen; is een synthetische stof die niet in de natuur voorkomt. Het heeft de laagste biologische activiteit en moet in de lever omgezet worden in vormen die biologisch actiever zijn. Deze omzetting is niet-efficiënt en sommige mensen hebben geen baat bij cyanocobalamine omwille van het ontbreken aan assimilatie of omzetting. (194-195) De vorm cyanocobalamine is echter wel noodzakelijk om Hydroxocobalamine te reguleren bij het verwijderen van overtollig stikstofmonoxide, en zou daarbij in supplementen met hydroxocobalamine moeten zitten. (176)

Wie heeft een tekort aan vitamine B12 en hoe komt dat?

In het algemeen kampen veel meer mensen met een vitamine B12-tekort dan vroeger werd aangenomen. Uit recente studies blijkt dat zo’n 78% van de ouderen een vitamine B12-tekort heeft. (196-197)

Het prikkelbare darmsyndroom (PDS), waaraan zo’n 77% van de ME/CVS-patiënten en 78% van de FM patiënten lijden, is een belangrijke oorzaak van een vitamine B12-tekort. (200) Dit lijdt ertoe na te denken over “wat kwam eerst, de kip op het ei”: Hebben ME/CVS-patiënten een B12 tekort omwille van PDS, of is PDS een gevolg van een cellulaire of neurologische beschadiging veroorzaakt door een B12 tekort?

Andere groepen met een hoog risico voor een B12 tekort zijn:

  • De mensen die zuurremmende of neutraliserende geneesmiddelen gebruiken (zoals omeprazole (Losec ®), esomeprazole (Nexiam ®) en andere (201-204);
  • Zij die geneesmiddelen gebruiken die de intestinale absorptie nadelig beïnvloeden (zoals Metformine, Questran, Chloramfenicol/chloromycetine) (205);
  • En mensen die een maagoperatie gehad hebben. (206-207)

Bacteriële overgroei van de dunne darm, die vaak voorkomt bij mensen met ME/CVS en laag maagzuur, is een predisponerende factor voor B12 tekort omdat de bacteriën zelf vitamine B12 gebruiken. (208-209)

De meest recente en verontrustende studies suggereren dat vitamine B12 tekort vaker voorkomt bij jong volwassenen dan eerder gedacht (210-211). Een studie wees uit dat vitamine B12 tekort gelijkaardig was in drie leeftijdsgroepen (26-49 jaar, 50-64 jaar, en 65 jaar en ouder), maar dat de eerste symptomen gewoon minder duidelijk waren bij de jongeren. Deze studie vond ook dat wie geen vitamine B12 bevattend supplement nam, twee keer zoveel kans had op een tekort dan de supplement gebruikers, ongeacht hun leeftijd. (210)

In de tweede plaats, in tegenstelling tot andere water oplosbare vitaminen, wordt B12 opgeslagen in de lever, de nieren en andere weefsels. Een tekort aan vitamine B12 ontstaat vaak sluimerend waardoor het te laat opgemerkt wordt. Kleine vitamine B12-tekorten worden in de helft van de bloedtesten over het hoofd gezien. (212-213)

Waarom veel mensen behoefte hebben aan vitamine B12 via supplementen.


De medische wetenschap was er ooit van overtuigd dat weinig mensen een vitamine B12 tekort hadden. Deze valse veronderstelling vloeide voort uit het feit dat vitamine B12 in het lichaam geproduceerd wordt door een normale, gezonde populatie van darmbacteriën.

Voeding is niet de belangrijkste bron van vitamine B12 in de meeste diëten. Vlees, melk, eieren, vis en schaaldieren bevatten de hoogste gehaltes aan vitamine B12, maar hiervan wordt slechts 50% geabsorbeerd in een gezonde darm. (215) Vegetarische bronnen van vitamine B12, zoals algen, zijn niet bio-beschikbaar en leveren geen belangrijke bijdrage tot het vitamine B12 gehalte uit voedsel. (216)

Verder wordt de opname belemmerd door onvoldoende maagzuur, het prikkelbare darmsyndroom en bacteriële overgroei van de dunne darm – aandoeningen die vaak voorkomen bij ME/CVS-patiënten.

Het U.S. Institute of Medicine raadt volwassenen ouder dan 50 aan om hun voorraad vitamine B12 te halen uit supplementen. (14)

Oraal of via injecties: welke vorm is de beste?

Hoewel vroeger vitamine B12 vooral toegediend werd via injecties, wordt nu in de reguliere geneeskunde algemeen aanvaard dat orale en sublinguale (onder de tong) vitamine B12 even efficiënt zijn als injecties bij de behandeling van pernicieuze anemie en andere aandoeningen met een B12 tekort. (214, 217-222)

Volgens de National Institutes of Health (NIH) is orale vitamine B12 suppletie uiterst veilig (221-22). Het is ook even efficiënt als injecties (14, 219-220) en goedkoop en handiger in vergelijking met injecties. (220)

All wegen leiden naar B12: Conclusies en aanbevelingen

Het lijden aan ME/CVS en verwante multisysteem aandoeningen is wijdverspreid met een verwoestend effect. Deze aandoening begint meer aandacht te krijgen, misschien door het activisme van de getroffenen en de inzet van ME/CVS onderzoekers en artsen. Recent onderzoek biedt ons nieuwe inzichten in de onderliggende mechanismen van deze ingewikkelde aandoeningen.

Onderzoek heeft aangetoond dat het stikstofoxide/ peroxynitrietmechanisme (NO-/ONOO-) en het methylatieproces twee belangrijke mechanismen zijn bij ME/CVS en andere multisysteem aandoeningen zoals fibromyalgie, de ziekte van Lyme, Meervoudige Chemische Sensitiviteit en Gulf War Syndrome. Een tekort aan Hydroxocobalamine en/of Methylcobalamine speelt een belangrijke rol in deze biologische processen.

Vitamine B12 (vooral Hydroxocobalamine en Methylcobalamine) heeft een positief effect, zonder gekende risico’s, op de gezondheidstoestand van ME/CVS-patiënten en patiënten die aan andere multisysteem aandoeningen lijden. Het is voor hen en patiënten met andere verwante multisysteem aandoeningen aangewezen om een supplement in te nemen die deze 2 belangrijke vormen van vitamine B12 bevat.

Bovendien, omwille van het balancerend effect dat cyanocobalamine op hydroxocobalamine heeft (176) en het beschermend en regeneratief effect dat adenysolcobalamine uitoefent op de myelineschede van de zenuwen (192-193), dienen deze vormen ook in beschouwing genomen te worden als belangrijk onderdeel van een volledig vitamine B12 supplement.
