Het Amerikaanse Ministerie van Volksgezondheid en Sociale Zaken (HHS) lijkt deels op haar schreden terug te keren om nieuwe “CVS”-criteria te doen ontwikkelen.

De plannen om het Institute of Medicine buiten iedereen om nieuwe criteria te laten uitwerken werden duidelijk toen een op de achtergrond gesloten contract met IOM openbaar werd.

Dit leidde tot een storm van protest van patiënten en een e-mail actie richting HHS. HHS lijkt nu deels op haar voornemen te zijn teruggekomen (meer info).

IOM gaat eigenhandig nieuwe diagnosecriteria voor CVS ontwikkelen (waarom?) en de NIH zal een workshop organiseren om tot nieuwe research-criteria te komen (waarom?).

Een groep van ME deskundigen tekent via een open brief grote bezwaren aan (zie onder) en pleit voor de invoering en verfijning van de Canadese Consensus-Criteria voor ME/CVS.

Wordt ongetwijfeld vervolgd.

Beste Secretaris Kathleen Sebelius,

Wij schrijven als biomedische onderzoekers en artsen met expertise in de ziekte myalgische encefalomyelitis / chronisch vermoeidheidssyndroom (ME/CVS) om u te informeren dat we een consensus bereikt hebben om de Canadese Consensus Criteria (CCC) (2003) in te voeren en te gaan gebruiken als de case definitie voor deze ziekte.

De 1994 Internationale Case Definitie (Fukuda et al., 1994), algemeen bekend als de Fukuda criteria, was de primaire case definitie voor ME/CVS gedurende bijna twee decennia. In de afgelopen jaren echter maakten de expert onderzoekers en artsen in toenemende mate gebruik van de CCC, omdat ze ingezien hebben dat de CCC een meer wetenschappelijke accurate beschrijving van de ziekte is.

De CCC is ontwikkeld door een internationale groep onderzoekers en artsen met aanzienlijke expertise in het ME onderzoek en behandeling en is gepubliceerd in een peer-reviewed journal in 2003 (Carruthers et al., Journal of Chronic Fatigue Syndrome, 2003). In tegenstelling tot de Fukuda definitie, bevatten de meer up-to-date CCC de uitgebreide wetenschappelijke kennis die voortkomt uit decennia van onderzoek. De CCC vereist bijvoorbeeld het symptoom van post-exertionele malaise (PEM), welke onderzoekers, artsen en patiënten als het hoofdkenmerk van de ziekte beschouwen, en die geen verplicht symptoom is onder de Fukuda definitie. De CCC werd bekrachtigd in de Primer for Clinical Practitioners die gepubliceerd werd door de International Association of Chronic Fatigue Syndrome / Myalgic Encephalomyelitis (IACFSME). Deze organisatie is de belangrijkste internationale professionele organisatie bezig met onderzoek en patiëntenzorg in ME/CVS.

De deskundige biomedische gemeenschap zal de case definitie blijven verfijnen en actualiseren conform de vooruitgang in wetenschappelijke kennis; de 2011 ME Internationale Consensus Criteria (Carruthers et al., Journal of Internal Medicine, 2011) kunnen hier bijvoorbeeld in beschouwing genomen worden. Als toonaangevende onderzoekers en artsen op dit gebied zijn we het er echter over eens dat er nu voldoende bewijs en ervaring is om nu de CCC aan te nemen voor onderzoek en klinische doeleinden, en wanneer dat niet gedaan wordt, zal dit het onderzoek belemmeren en de patiëntenzorg schaden. Deze stap zal onze inspanningen vergemakkelijken om biomarkers te definiëren, die gebruikt zullen worden om verder de case definitie te verfijnen in de toekomst.

We dringen er sterk op aan bij het Departement of Health and Human Services (HHS) om ons voorbeeld te volgen door gebruik te maken van de CCC als de enige case definitie voor ME/CVS bij alle activiteiten van het Departement met betrekking tot deze ziekte.

Bovendien dringen we er sterk op aan om pogingen achterwege te laten om groepen zoals de Institute of Medicine (IOM) te benaderen die niet de nodige expertise hebben om “klinische diagnostische criteria” voor ME/CVS te ontwikkelen.

Aangezien de deskundige ME/CVS wetenschappelijke en medische gemeenschap een case definitie ontwikkeld en goedgekeurd heeft voor onderzoek en klinische toepassingen, is deze poging onnodig en een verspilling van het schaarse belastingsgeld dat veel beter besteed zou worden aan financiering van onderzoek naar deze ziekte. Erger nog, door deze poging dreigt de ME/CVS wetenschap achteruit te gaan door non-experten aan te moedigen een case definitie te ontwikkelen voor een complexe ziekte waarover zij niet goed geïnformeerd zijn.

ME/CVS patiënten die al tientallen jaren geïnvalideerd zijn door deze verwoestende ziekte moeten het onderzoeksdomein zien vooruitgaan en er is geen tijd meer te verliezen. We zijn van mening dat onze consensus beslissing over een case definitie voor deze ziekte vooruitgang in gang zal zetten en tot veel snellere vooruitgang zal leiden in onderzoek en zorg voor ME/CVS-patiënten. We kijken uit naar deze versnelde vooruitgang en staan klaar om samen met u te werken om de wetenschappelijke kennis van de pathofysiologie van deze ziekte te verhogen, medische professionals voor te lichten, meer doeltreffende behandelingen te ontwikkelen en uiteindelijk een remedie te vinden.
